Prepare the file and documents required :

  • A copy of grade 5 student report card for junior high school registrant.
  • A copy of grade 8 student report card for senior high school registrants.
  • A copy of birth certificate
  • A copy of family card
  • A copy of parents ID card
  • A copy of health insurance Card

Syarat Pendaftaran

 

Syarat pendaftaran adalah  sebagai berikut:

  1. Membayar Biaya Pendaftaran Rp.500.000,- ke No. Rekening 1330016008153 a.n Yayasan Khazanah Cahaya Iman (Bank Mandiri).
  2. Murid kelas VI SD/MI dan kelas IX SMP/MTs negeri maupun swasta.
  3. Mengisi formulir pendaftaran Formulir bisa diambil di kampus SMP-SMA Cahaya Rancamaya atau download di Download Formulir.
  4. Menyerahkan foto copy rapor yang dilegalisir dari sekolah asal. Rapor kelas  4 dan 5 (bagi calon siswa SMP), kelas 7 dan 8 (bagi calon siswa SMA).
  5. Menyerahkan foto copy Akta Kelahiran calon siswa (1 lembar).
  6. Menyerahkan foto copy Kartu Keluarga (1 lembar.)
  7. Menyerahkan foto copy KTP Orang Tua (1 lembar).
  8. Menyerahkan foto copy kartu BPJS / asuransi kesehatan.
  9. Menyerahkan pas foto warna ukuran 3×4 sebanyak 4 lembar.
